ADMISSIONS REQUIREMENTS

1.    Original Form 138 / School Report Card (for transferees only)
– With English translation for non-Filipino applicants
– Must be authenticated by their country’s embassy for students who studied in schools outside Oman and the Philippines
– Must bear the official school stamp

2.    Form 137 / Transcript of Records (for transferees only)
– Complete academic records required for non-Filipino applicants
– For Filipino transferees, a requisition form for Form 137 is available at the Registrar’s Office, c/o Ms. Eden V. Flores

3.    Original Certificate of Good Moral Character / Honorable Dismissal (for transferees only)
– Issued by the principal of the former school
– Original copy is required
– Must bear the official school stamp

4.    Certificate of Transfer (for transferees only)

5.    Clear Copy of Passport (ID Page) and Valid Visa
– For both parents and child
– Printed copy of eVisa is required

6.    Clear Copy of Birth Certificate
– PSA Birth Certificate for Filipino applicants
– With English translation for non-Filipino applicants

7.    Clear Copy of Oman Residence Card (back-to-back copy)
– For both parents and child

8.    Clear Copies of Health Records / Immunization Records

9.    Three (3) Passport-Size Photos
– With blue background
– Recent and identical

10.    For Omani Nationals:
– Submit the Permit to Study in a Non-Omani School
– This permit should be requested from the Ministry of Education – Oman
